Required documents for new students

What information do you gather on your new students? What forms do you require of them?

Let's say I have the following.

Basic info including contact & emergency contact info.
Release of liability waiver (duh, we all have to do that one)

What else?

Does anyone require proof of medical insurance? I've seen some forms that ask for prior training info, if it's not Kajukenbo, what use is that to you?

In my school we ask for a medical certificate. This is to know if the person have any condition.
 We also ask if the student have any backround for some reasons
1 This student usually advance more quickly in the ranks because he knows the basics postures as blocks, kicks, positions
2. If this student is a black belt in other style you would not put him to spar which a yellow rank student (example) or you as a teacher will be more carefull when this sparring is going.
