Release of Liability

I'm on a military base teaching a kajukenbo kids class and have been for about 6 months and everything is supposed to be covered by the youth center im teaching at but just recently the Security Officer here is telling me that to continue teaching i need to have a release of liability form go through them and then JAG. I have no clue how to build one of these any help is greatly appriciated. Thank you.

Yes the waiver that Sigung Jeff mentioned was designed by one of my black belts (Sifu Virgil).  At one of Sigung Trent's Norhwest Kajukenbo Gatherings Sifu Virgil gave a talk and showed examples of the problems that teachers can run into.  It was really some great information.  As Sigung Jeff suggested contact Sigung Trent as I know he has a copy readily available.

Sifu Dean (KDMA) was teaching on a navy base in their base gym while he was on active duty, so he may have some inside military views on this as well.
Anyone that has been in the military knows that the military have their own way of looking at things regardless of how the public runs things.
